Iron, Ferritin, and the Tiredness Nobody Investigates

You've been tired for months.

Not sleepy. Tired in a way sleep doesn't fix. The hill on your usual walk takes something out of you now. You're winded on the second flight of stairs you climb every day. Your legs won't settle at night. There's more hair in the drain than there used to be.

So you did the sensible thing and had bloodwork done. Everything came back normal.

Nobody checked your ferritin.

You're not imagining this, and you're not unusual. Iron deficiency is the most common nutrient deficiency in the world. The version that gets missed is the one that isn't anemia yet. Your hemoglobin can sit squarely in range while your iron stores are effectively empty.

Before you accept another year of "it's probably just your age," here's what's actually happening and the number to ask for.

Hemoglobin and Ferritin Answer Different Questions

Hemoglobin measures the iron circulating in your red blood cells right now. Ferritin measures the protein that stores iron, so it tells you what you hold in reserve.

Which one did your doctor order?

Your body protects circulation first. When losses outpace intake, it spends down the reserve to keep hemoglobin normal. That is paying the bills out of savings while the checking balance still looks fine.

By the time hemoglobin drops, the savings have been gone a long while.

Why Low Iron Feels Like So Many Different Things

Iron isn't only about carrying oxygen, though that alone explains the breathlessness.

Iron is also a cofactor inside your mitochondria, the structures in your cells that produce energy. It's required to convert thyroid hormone into its active form. It's required by the enzyme that builds dopamine.

So when stores run low, the trouble spreads well past your breathing. You get flat mood, a fogged afternoon, sluggish recovery, restless legs, and diffuse hair shedding. That shedding is called telogen effluvium, where follicles drop into a resting phase early.

That scatter is why it gets missed. No single symptom points at iron. All of them together do.

Why This Matters Beyond Feeling Tired

I want to be clear about the stakes, because "tired" sounds minor.

When oxygen delivery suffers, training gets harder, so you train less. You lift lighter, so you lose the stimulus that maintains muscle and bone. Recovery drags, so you skip more sessions.

Low iron costs you more than a few flat months. It removes the inputs that protect your strength and your independence for decades.

And you'll blame yourself for it. That's the part that genuinely frustrates me. You may simply be under-fueled at the cellular level.

The Signs Worth Taking Seriously

Breathlessness on a hill that used to be nothing. A heart rate that spikes higher than the effort deserves. Hair in the shower drain. Legs that crawl the moment you lie down. Cold hands in a warm room. A hard stop in your energy by mid-afternoon. And the strangest one, craving and chewing ice.

If you're nodding along to several of these, you don't need to guess. You need numbers.

Two Reasons This Happens, and You Can Have Both

Iron trouble comes from two directions. You're either losing more than you take in, or not absorbing what you eat.

Losing it looks like heavy or long periods, the flooding cycles of perimenopause, frequent blood donation, endurance running, or bleeding in your digestive tract that you don't know about.

Not absorbing it looks like eating far less overall, which is the entire mechanism of a GLP-1. It also looks like a plant-forward diet without strategic pairing, long-term acid reducers, celiac disease, or coffee with every meal.

Here's the catch: you can have both. The perimenopausal runner who drinks coffee with breakfast is losing iron and blocking absorption at once.

What to Ask For, Specifically

Ask your physician for the full picture rather than a blood count alone:

  • Ferritin, not just hemoglobin
  • Complete blood count, including MCV
  • Serum iron, total iron binding capacity, and transferrin saturation
  • CRP, because inflammation raises ferritin and can mask a deficiency
  • B12 and folate
  • A thyroid panel, including TSH
  • Vitamin D, since the symptoms overlap

Timing changes what those numbers mean. Don't take an iron supplement in the 24 to 48 hours before your draw. Don't get labs done the morning after a hard session or a long day in the heat. Both raise inflammatory markers, and ferritin rises with them. A ferritin drawn the morning after a hard ride can read comfortable when your stores are not.

On the number itself, let me be direct. Most lab ranges start their "normal" around 10 to 15, and that floor was set to catch anemia, not to define adequate. I want to see ferritin comfortably above 50, and closer to 70 to 100 when hair shedding or restless legs are in the picture. Your physician interprets your result. Your job is to get it in hand.

What to Do About It

Get the number before you take the pill. Iron is not a supplement to take on a hunch. Too much iron is its own problem. I see this mistake constantly. Someone reads an article, buys a bottle, feels no different, and never learns what their number was.

Know which iron you're eating. Heme iron, the kind in beef, liver, oysters, sardines, and dark poultry, is absorbed several times more efficiently than the non-heme iron in lentils, tofu, spinach, and fortified grains. Spinach is not an iron plan. If most of your iron comes from plants, pair it with vitamin C at the same meal.

Move the blockers away from the meal. Coffee and tea contain polyphenols that bind non-heme iron in your gut, and calcium competes with it directly. Drink your coffee between meals rather than with them. Take calcium at a different time of day than your iron pill. This one costs you nothing.

If you supplement, do it the way absorption works. A dose of iron raises hepcidin, the hormone that regulates how much iron your gut lets in, and it stays elevated for roughly a day. That's why one daily dose, or even every other day, is often absorbed better than split doses. Take it with vitamin C and away from coffee, dairy, and calcium.

Refilling stores takes months, not weeks. Plan on a recheck, not a feeling. Your prescriber sets the dose and form, and if oral iron isn't tolerated, ask about IV iron.

No supplement replaces finding out why the iron is leaving. Pick one thing this week: move the coffee, or book the draw.

When This Needs a Physician, Not a Plan

Call your doctor if your periods soak through protection hourly, run longer than seven days, or pass large clots. Call if you see blood in your stool, or stools that look black and tarry.

And if you're a man, or a woman past menopause, low iron is not something to supplement and move on from. The deficiency is a finding, not a diagnosis, and it warrants a look at your digestive tract. I don't diagnose and I don't prescribe. What I do is make sure the right question gets asked at your appointment.

A Better Scorecard

Stop scoring this by how tired you feel. Fatigue is a terrible instrument. You adapt to it, then you talk yourself out of it.

Score it with three things instead. Your ferritin trend across two draws a few months apart. Your effort on one repeatable benchmark, the same hill or the same stairs. And your recovery, meaning how you feel the day after you train.

The Bottom Line

You can spend another year going to bed earlier, adding a third coffee, and deciding this is simply what getting older feels like. Or you can spend one blood draw finding out whether your reserves are empty.

Low ferritin is one of the most correctable causes of exhaustion I come across. It's also one of the most overlooked, because the test that finds it doesn't get ordered unless somebody asks for it.

Ask for it. You deserve to know why you're tired.
