Supplements I Actually Recommend, and the Ones I Don't

Let me describe a cabinet I see almost every week.

There are fourteen bottles in it. A few expired last year, two arrived because a podcast named them in January, and one gets taken every morning by someone who can't say what it's for.

Somewhere behind the greens powder, the thing that would actually close a gap is either missing or sitting at a dose too low to matter.

You're not being careless. You're doing what the shelf asks you to do.

Supplements in the US are not reviewed for safety or effectiveness before they go on sale. A company writes the label, the bottle reaches the store, and you're left to sort out what's real. You're doing the manufacturer's homework at seven in the morning.

So let's talk about what a supplement can do, which ones earn a place in your cabinet, and which ones I would take out.

What a Supplement Can and Cannot Do

A supplement closes a gap. That is the entire job.

There are three gaps worth closing. A measured deficiency, confirmed on a lab. A demand your diet cannot practically meet, like protein at the intake your muscle needs after 40. Or a depletion created by something else in your life: a medication, a restricted diet, or four months of the year with no usable sun.

If a bottle doesn't map to one of those three, it's decoration.

A supplement also cannot outrun the thing creating the gap. If you're sleeping five hours, under-eating protein, and never lifting anything heavy, no capsule reaches that. It's like topping off the oil in a car with a leak. You aren't wrong to add oil. You're just not fixing the leak.

The Signs Your Cabinet Needs an Audit

You take something you can't explain, because the reason arrived in a podcast and left again. You've never had a level drawn for anything in there, so you're guessing at the gap and the dose at the same time. You take iron without a ferritin result.

You take a multivitamin plus three single nutrients that already sit inside it. Your magnesium is oxide, the cheapest form and the least absorbed. Your fish oil says 1,000 milligrams on the front and you've never turned the bottle over. Two things expired during a year you kept taking the rest.

If several of those describe your cabinet, it's worth an hour of your attention.

The Ones I Actually Recommend

Vitamin D3 earns its place for almost everyone. It behaves less like a vitamin and more like a hormone, and one of its jobs is governing how much calcium you absorb at the gut. Without enough of it, much of the calcium in your food passes straight through.

The doses I see used most often run 2,000 to 4,000 IU daily, and the right number for you depends on your level and on who is managing it. Take it with the meal that has the most fat in it, since it needs fat to be absorbed. Black coffee is not a delivery vehicle.

There's a seasonal catch worth knowing. Summer is the one stretch of the year when your skin makes meaningful vitamin D at northern latitudes. A level drawn in late summer is your high-water mark, not your average. If summer reads borderline, winter is unlikely to read better.

Magnesium is the second one. It's a cofactor in hundreds of enzymatic reactions, including the ones that move energy inside your cells and the ones that let a contracted muscle release.

Form matters more than brand. Glycinate is the gentle version and suits evening dosing. Citrate draws water into the colon, the right pick when things aren't moving. Oxide is cheap, poorly absorbed, and what's in most bargain bottles. The bottles you'll find land between 200 and 400 milligrams a serving.

One testing note, because this trips people up. Magnesium on a routine panel can read normal while your tissue stores are low, since your body pulls from bone to hold blood levels steady. RBC magnesium tells you more. Ask for it by name.

Creatine monohydrate belongs in far more cabinets than it occupies. It keeps phosphocreatine available inside your muscle, which is how you regenerate energy during short, hard efforts. More available energy means better repetitions, and repetitions are the signal that keeps muscle on your frame.

Three to five grams daily of plain monohydrate is the amount most of the research uses, and timing does not appear to matter much.

The mistake I see is assuming creatine belongs to twenty-five-year-old men. The people with the most to gain are women in midlife and adults past sixty. This is about getting off the floor at 80 without using your hands.

Omega-3s, with a label check. If you eat fatty fish twice a week, food has this handled. If you don't, a fish oil or algae-based product is reasonable.

Read the back panel, not the front. A bottle advertising 1,000 milligrams describes the weight of the oil, not the EPA and DHA inside it. Look for at least one gram of the two combined.

And protein powder, which doesn't belong in this category at all. It's food in an inconvenient shape. The range I work toward with clients is 0.7 to 1.0 gram of protein per pound of goal body weight. If you're landing 30 grams short of that most days, one scoop solves an arithmetic problem. That isn't optimization. That's groceries.

The Ones I Would Take Back Out

Greens powders standing in for vegetables. A scoop delivers concentrated plant material and almost none of the fiber or potassium volume of actual produce. On top of vegetables they're fine. In place of vegetables they're an expensive way to skip the salad.

Anything listing a proprietary blend. When a panel groups eight ingredients under one combined number, you cannot know how much of any single one you're taking. A blend is how a company puts a trace of the expensive ingredient behind a pile of the cheap one.

Metabolism and fat-burning products. Most are stimulants stacked on stimulants. They lift your resting heart rate and dismantle the sleep everything else depends on.

Iron without a ferritin result. This is the one where guessing can genuinely harm you, because iron accumulates and your body has no efficient way to unload it. Ferritin reflects your stores. Hemoglobin often looks fine until you're well down the road.

Here's the catch, and it applies to both lists. Well studied means studied in a particular group, at a particular dose, against a particular starting level. Whether that study describes you depends on your labs, your medications, and your kidneys. That is a conversation for the person who holds your chart.

How to Read a Label in Thirty Seconds

Three checks, in this order. Look for a third-party verification mark such as NSF or USP. It means an outside lab confirmed the bottle contains what the label claims. Nobody verifies that before it reaches the shelf, so it's the closest thing to a guarantee you get.

Then check the serving size. The impressive number on the front is often per two or three capsules, which quietly multiplies your dose and your cost.

Then check the form and, for minerals, the elemental amount. Six hundred milligrams of a magnesium compound is not six hundred milligrams of magnesium.

When to Talk to Your Doctor First

I'm a dietitian. I don't diagnose and I don't prescribe, and several of these belong to your physician first.

Call before adding vitamin K2 or fish oil if you take a blood thinner. Call if you take thyroid medication, since calcium and iron interfere with its absorption and need hours of separation. Call before adding magnesium or potassium if you have kidney disease. Call if you're pregnant.

And call if a product claims to influence blood sugar while you're on a GLP-1, insulin, or another glucose-lowering medication. Stacking those effects is how people end up lightheaded in a parking lot.

Your doctor is also the one who decides which levels get drawn, at what dose anything gets started, and when it's worth checking again. My job is to tell you which questions are worth asking.

The Bottom Line

You can spend real money every month and still be deficient in the thing limiting you. That's the trade worth avoiding. Supplements are the smallest lever in the room. They pay off only when the large ones are moving: protein, resistance training, sleep, and time outside.

When someone brings me their cabinet, we subtract before we add. We look at the labs, then the plate, then the training, then the bottles. Most people end up taking fewer things at better doses.

You don't need to overhaul everything at once. Pull every bottle onto the counter this week and answer one question for each: which gap is this closing? Anything you can't answer goes into a box on the floor for thirty days, not the trash. Notice which ones you go looking for.

Which supplements are actually worth taking?

A supplement only earns a place when it closes a real gap: a measured deficiency, a demand your diet cannot practically meet, or a depletion caused by a medication or a sunless season. The shortlist I see closing real gaps is vitamin D3, magnesium, creatine monohydrate, omega-3s if you rarely eat fish, and protein powder when the arithmetic falls short.

Do I need blood work before starting a supplement?

For several of them, yes. Iron is the clearest case, since it accumulates and guessing can harm you, so ask for ferritin rather than hemoglobin. Vitamin D and B12 are worth measuring before you start, and your doctor is the one who decides when a recheck makes sense. For magnesium, request RBC magnesium, because a routine serum level can read normal while tissue stores are low.

Are greens powders a substitute for vegetables?
